For pediatricians, therapists, schools, and consultants: when a family needs school and therapy together, or a comprehensive evaluation, refer to Black Bear Academy. A team member will follow up and coordinate with a signed release.

Use the form, or call us. Share as much or as little as the family is comfortable with.
We contact you, then the family
A team member contacts you first to arrange how to reach the family, then follows up with the family to listen and explain next steps. You can also give the family our number: (773) 244-0700.
We coordinate records with a signed release
With a signed release, we coordinate records and talk with you so nothing gets lost in the hand-off.
The family gets an integrated plan
The family receives an integrated plan across school, therapy, and evaluation, or an honest referral elsewhere if we are not the right fit.
